Parents please use this section of the website to submit all applications, you can download the application fill it out and sign it then upload it here. We will be updating each team as the applications and payments are recieved. If a team is filled up for an age group we will post it.

PHYSICAL FORM

Fill the form out completely. 

Do not electronically sign the form!
– Select file, Save as, and rename as your athletes Last name, First name and then save the file.
– Email the completed form, signupcyfa@gmail.com with your athletes name as the subject line.  

– click make a payment, it will take you to our pay-pal acct 
 (min down payment of $100, the rest to be paid prior to 7/1/2020)
    make sure to put your children’s name in the PAYMENT
